1. 深圳市国际招标有限公司(以下简称“招标机构”)受招标人委托,邀请合格投标人就深圳大学附属医院(原学府医院)的下列货物和有关服务提交密封投标:

2. 有意向的合格投标人可在深圳市国际招标有限公司得到进一步的信息和查阅招标文件。
3. 投标人资格:
(1)投标人必须是来自中华人民共和国或是与中华人民共和国有正常贸易往来的国家或地区的法人或其他组织,并在中国国际招标网(www.chinabidding.com)注册成会员;须提供营业执照或企业注册证明复印件。
(2)国内投标人须为深圳市政府采购中心注册的供应商;(由招标机构在深圳市政府采购中心系统中查询,投标人无需提供证明材料)
(3)投标人必须是所投产品的制造商或制造商有效授权的合法代理商,并提供设备制造商专项授权书或代理证书复印件(投标人是设备制造商的除外),并加盖投标人公章。一家制造商只能授权一家投标人投标。
(4)国内投标人需具有《中华人民共和国医疗器械生产许可证》或《中华人民共和国医疗器械经营企业许可证》(提供证书复印件并加盖投标法人公章)。
(5)投标人所投设备必须具有《中华人民共和国医疗器械注册证》(提供证书复印件并加盖投标法人公章)。
(6)投标人必须提供由设备制造商或授权的中国总代理签署的合法有效的保修、维修承诺函。
(7)国内投标人近三年内无行贿犯罪记录。(由深圳市政府采购中心定期向市人民检察院申请对政府采购供应商库中注册有效的供应商进行集中查询,投标文件中无需提供证明材料)
4.有意向的投标人可从2017年6月8日至2017年6月15日止,登录“深圳市国际招标有限公司电子招标采购平台”(http://www.sztc.com/)注册会员、报名、购买并下载招标文件,本招标文件每套售价人民币500元或80美元,售后不退。投标人登录平台购买招标文件并完成“添加购标订单”操作并进行付款,付款成功并经平台财务审核通过后即可在平台点击下载招标文件。
5. 所有投标书应于2017年6月29 日15:00(北京时间)之前递交到深圳市国际招标有限公司(深圳市罗湖区嘉宾路2018号深华商业大厦裙楼6楼)633开标室。
6. 定于2017年6月29 日15:00 (北京时间),在深圳市国际招标有限公司(深圳市罗湖区嘉宾路2018号深华商业大厦裙楼6楼)633开标室。届时请参加投标的代表出席开标仪式。

1. Shenzhen International Tendering Co. Ltd. (hereinafter referred to as "tendering agent") entrusted by the tenderer, invite eligible bidders to the Affiliated Hospital of Shenzhen University (formerly seats hospital) of the goods and related services to submit sealed bids:

3. Qualifications and performance standards:
(1) the bidder must be from People's Republic of China or legal person and the people's Republic of China Normal Trade between countries or regions or other organizations, and in the China international tendering (www.chinabidding.com) registered members; to provide business license or business registration certificate copy.
(2) the domestic bidder shall be a registered supplier of the Shenzhen municipal government procurement center; (the bidder shall not be required to provide certification materials in the inquiry system of the Shenzhen municipal government procurement center)
(3) the bidder must be cast by the product manufacturer or the manufacturer effectively authorised agents, and provide equipment manufacturer special authorization agent or a copy of the certificate (except the bidder is equipment manufacturers), and stamped with the official seal of bidders. A manufacturer can only authorize a bidder to tender.
(4) the domestic bidder has "People's Republic of China medical equipment production license" or "People's Republic of China medical devices business license" (provide copy of the certificate of legal tender and stamped with the official seal).
(5) the equipment to be delivered by the bidder must have the People's Republic of China medical device registration certificate (a copy of the certificate and the official seal of the tender).
(6) the bidder must provide the lawful and effective warranty and maintenance undertaking signed by the equipment manufacturer or the authorized Chinese general agent.
(7) domestic bidders have no record of bribery in the past three years. (by the Shenzhen municipal government procurement center regularly apply to the Municipal People's Procuratorate for centralized inquiry on the valid suppliers registered in the government procurement suppliers' library, without any certification materials in the tender documents)
