1、深圳市国际招标有限公司（以下简称“招标机构”）受招标人委托，邀请合格投标人就深圳市人民医院图像引导放射治疗系统采购的下列货物和有关服务提交密封投标：

3、资格证明文件和业绩：
1) 投标人必须是来自中华人民共和国或是与中华人民共和国有正常贸易往来的国家或地区的法人或其他组织，并在中国国际招标网（www.chinabidding.com）注册成会员；（须提供营业执照或企业注册证明复印件）
2) 国内投标人须为深圳市政府采购中心注册的供应商；（由招标机构在深圳市政府采购中心系统中查询，投标人无需提供证明材料）
3) 参与本项目政府采购活动前三年内，在经营活动中没有重大违法记录；（必须按照招标文件格式IV-11. 声明函格式内容提供书面声明函）
4) 参与本项目政府采购活动时不存在被有关部门禁止参与政府采购活动且在有效期内的情况；（必须按照招标文件格式IV-11. 声明函格式内容提供书面声明函）
5) 本项目不接受联合体投标人，不允许分包；
6) 若所投产品为进口，则投标人必须提供由设备制造商或授权的中国总代理签署的合法有效的保修、维修承诺函；（提供相关证明复印件,原件备查）
7) 若投标人为所投产品的生产企业，必须提供《医疗器械生产企业许可证》，且生产范围包含该产品；若投标人为所投产品的代理商或授权供应商，必须提供《医疗器械经营企业许可证》，且经营范围包含该产品；（提供复印件，原件备查）
8) 投标人必须提供所投产品的《医疗器械注册（备案）证》的复印件，原件备查（开标时，该证应在有效期内；若不在有效期内，则需提供该证和所投产品在该证有效期内生产的药监局出具的证明文件）。
注：投标人必须到中国国际招标网注册(www.chinabidding.com)。
4、招标文件获取：
（1）请于2018年12月28日上午9：00至2019年1月7日下午17：00（北京时间）登陆深圳市国际招标有限公司网站，下载招标文件（订购前须确保在www.sztc.com网站已注册会员，如未注册，请登陆http://www.sztc.com/gyssc/17414.jhtml页面了解如何免费注册会员）。
（2）本项目免招标文件订购费，用户仅需购买电子招投标服务即可参与项目。
（3）电子招投标服务收费：人民币2500元，支付成功后由平台方开具电子发票，投标人如需纸质发票，系统将在支付成功后由平台方于一周内将纸质发票寄出，邮费到付。

1. Shenzhen International Tendering Co., Ltd. (hereinafter referred to as “tendering agency”) is entrusted by the tenderee to invite qualified bidders to submit sealed bids for the following goods and related services purchased by Shenzhen People's Hospital Image Guided Radiation Therapy System:

2. Invited qualified bidders can obtain further information and consult the bidding documents at Shenzhen International Tendering Co., Ltd.
3. Qualification documents and performance:
1) The bidder must be a legal person or other organization from the People's Republic of China or a country or region with normal trade with the People's Republic of China, and be registered as a member on the China International Bidding Network (www.chinabidding.com); Copy of business license or business registration certificate)
2) Domestic bidders must be registered with the Shenzhen Municipal Government Procurement Center; (inquiries by the tendering agency in the Shenzhen Municipal Government Procurement Center System, bidders are not required to provide supporting materials)
3) There are no major illegal records in the business activities during the three years prior to participating in the government procurement activities of the project; (a written statement must be provided in accordance with the format of the bidding documents IV-11.
4) There is no situation in the government procurement activities of the project that is prohibited from participating in government procurement activities and is within the validity period; (a written statement must be provided in accordance with the format of the bidding documents IV-11.
5) The project does not accept consortium bidders, and subcontracting is not allowed;
6) If the product to be imported is imported, the bidder must provide a legal and valid warranty and maintenance commitment letter signed by the equipment manufacturer or the authorized Chinese general agent; (provide a copy of the relevant certificate, the original is available for reference)
7) If the bidder is the manufacturer of the product to be invested, the Medical Device Manufacturing Enterprise License must be provided and the production scope includes the product; if the bidder is the agent or authorized supplier of the product to be invested, the medical device operation must be provided. Enterprise License, and the business scope includes the product; (a copy is provided, the original is available for reference)
8) The bidder must provide a copy of the “Medical Device Registration (Registration) Certificate” of the product to be invested. The original is for reference (the certificate should be valid within the validity period; if it is not within the validity period, the certificate and the product to be invested shall be provided) A certificate issued by the SFDA during the validity period of the certificate).

4. Acquisition of bidding documents:

(2) The project is free of bidding documents and the user only needs to purchase the electronic bidding service to participate in the project.

6. At 9:30 (Beijing time) on January 18, 2019, the opening bid of the 621 opening bidding room of Shenzhen International Tendering Co., Ltd.
