1、深圳市国际招标有限公司（以下简称“招标机构”）受招标人委托，邀请合格投标人就16排计算机断层扫描设备的下列货物和有关服务提交密封投标：

2、有意向的合格投标人可在深圳市国际招标有限公司得到进一步的信息和查阅招标文件。
3、资格证明文件和业绩：
1）、投标人必须是来自中华人民共和国或是与中华人民共和国有正常贸易往来的国家或地区的法人或其他组织，并在中国国际招标网（www.chinabidding.com）注册成会员；须提供营业执照或企业注册证明复印件。
2）、国内投标人须为深圳市政府采购中心注册的供应商；（由招标机构在深圳市政府采购中心系统中查询，投标人无需提供证明材料）
3）、投标人必须是所投产品（16排计算机断层扫描设备）的制造商或制造商有效授权的合法代理商，并提供设备制造商专项授权书或代理证书复印件（投标人是设备制造商的除外），并加盖投标人公章。一家制造商只能授权一家投标人投标。
4）、国内投标人需具有《中华人民共和国医疗器械生产许可证》或《中华人民共和国医疗器械经营企业许可证》（提供证书复印件并加盖投标人公章）。
5）、投标人所投设备（16排计算机断层扫描设备）必须具有《中华人民共和国医疗器械注册（备案）证》（注册证须含附件：医疗器械产品生产制造认可表或医疗器械产品注册登记表）（提供证书复印件并加盖投标人公章）（开标时，该证应在有效期内；若不在有效期内，则需提供该证和所投产品在该证有效期内生产的药监局出具的证明文件）。
6）、参与本项目政府采购活动前三年内，在经营活动中没有重大违法记录（必须按照招标文件格式IV－11. 声明函格式内容提供书面声明函）；
7）、参与本项目政府采购活动时不存在被有关部门禁止参与政府采购活动且在有效期内的情况；（必须按照招标文件格式IV－11. 声明函格式内容提供书面声明函）
8）、本项目不接受联合体投标，不允许分包。

4、招标文件获取：

1. Shenzhen International Tendering Co., Ltd. (hereinafter referred to as “Tendering Agency”) is entrusted by the tenderee to invite qualified bidders to submit sealed bids for the following goods and related services of 16 rows of computed tomography equipment:

3. Qualification documents and performance:
1) The bidder must be a legal person or other organization from the People's Republic of China or a country or region with normal trade with the People's Republic of China, and be registered as a member on the China International Bidding Network (www.chinabidding.com); A copy of the business license or business registration certificate.
2) Domestic bidders must be registered with the Shenzhen Municipal Government Procurement Center; (inquiries by the tendering agency in the Shenzhen Municipal Government Procurement Center System, bidders are not required to provide supporting materials)
3) The bidder must be a valid agent authorized by the manufacturer or manufacturer of the product (16 rows of computed tomography equipment) and a copy of the special authorization letter or agency certificate of the equipment manufacturer (the bidder is equipment manufacturing) Except for the business, and stamp the official seal of the bidder.
